With music by David Bryan (of Bon Jovi fame) and a book and lyrics by Joe DiPietro, The Toxic Avenger: The Musical reimagines the ultra-violent, satirical movie into an over-the-top, campy stage production filled with humor, romance, and environmental activism. First developed in the mid-2000s, the show takes the core elements of the original film—mutant anti-hero Toxie, his blind love interest Sarah, and the corrupt forces polluting Tromaville—and turns them into a high-energy, tongue-in-cheek musical spectacle.

The musical had its world premiere at the George Street Playhouse in New Brunswick, New Jersey, in October 2008. This initial run, directed by John Rando, received positive feedback, leading to further refinements before an Off-Broadway debut. On April 6, 2009, The Toxic Avenger opened at New World Stages in New York City, featuring a cast that included Nick Cordero as Toxie, Nancy Opel in multiple roles (including the villainous mayor), and Sara Chase as Sarah. The production embraced a fast-paced, fourth-wall-breaking style, with a small ensemble cast playing multiple characters, emphasizing its campy and irreverent nature. The Off-Broadway run lasted until January 2010, with over 300 performances, and earning strong reviews and winning the Outer Critics Circle Award for Best Off-Broadway Musical.

Following its Off-Broadway success, the musical was licensed for regional productions, including a Canadian premiere in Toronto in 2009 and various productions across the United States. In 2016, The Toxic Avenger made its London debut at the Southwark Playhouse, directed by Benji Sperring. The London production was well-received, praised for its energetic performances, outrageous humor, and catchy rock score. The show later transferred to the Arts Theatre in the West End for a limited run in 2017, solidifying its reputation as a cult favorite in musical theater circles.

Musically, The Toxic Avenger blends rock, pop, and Broadway styles, with standout songs like "Who Will Save New Jersey?," "Hot Toxic Love," and "The Legend of the Toxic Avenger." David Bryan's rock background infused the score with an infectious, driving energy, while Joe DiPietro's witty lyrics maintained the irreverent, politically incorrect humor of the source material. The show's small cast and quick costume changes further contributed to its comedic charm, making it a fast-paced, high-energy experience for audiences.

Despite not making the leap to Broadway, The Toxic Avenger: The Musical remains a beloved cult hit, continuing to be performed in regional and international theaters.
